Correlation study on hippocampal metabolic alterations in patients with temporal lobe epilepsy before and after treatment ①

Abstract Objective: To investigate the hippocampal metabolic alterations of patients with temporal lobe epilepsy before or after the treectment. Methods: 55 temporal lobe epilepsy patients and 20 healthy controls were enrolled, and the changes of hippocampus NAA/ (Cr+Cho) value in patients with temporal lobe epilepsy were compared before or after treatment. Results: Before treatment, the NAA/ (Cr+Cho) value in the affected side hippocampus in the epilepsy group was lower than that of the control group, and the difference was statistically significant (P<0.05). There was no significant difference of the NAA/ (Cr+ Cho) value of the contralateral hippocampus compared with the control group (P>0.05). After treatment, there were no statistical differences of the respective NAA/ (Cr+Cho) values between the affected side and contralateral hippocampus in the epilepsy group and those of the hippocampus in the control group (P> 0.05). Compared with that before treatment, NAA/ (Cr+Cho) value in the affected hippocampal head, hippocampus body in the epilepsy group was increased (P<0.05); NAA/ (Cr+Cho) value of the affected hippocampal head in the epilepsy inefficiency treatment group was lower than that in the effective group after treatment (P<0.05).The frequency of seizures was negatively correlated with the NAA/ (Cr+Cho) value of the affected hippocampus in the epilepsy group before treatment. Conclusion: The abnormal metabolism of hippocampus in patients with epileptic can be restored to normal level after effective antiepileptic treatment, and the metabolic alterations of hippocampal head may be the most sensitive to epileptic control.
